Our Mission
Our mission is to provide ethical rehabilitation services to people with disabilities in order to support the rehabilitation plan. In the event an injured worker is not able to return to their date of injury position, we provide client centered rehabilitation services to return them to suitable gainful employment, as well as comprehensive vocational evaluations.

Qualified Rehabilitation Services
As a Qualified Rehabilitation Consultant (QRC) we provide vocational services to injured worker’s in the State of Minnesota. We are a private neutral party that provides rehab benefit services at no cost to the injured worker.

Adam TeBrake
Adam is a proud alum of Gustavus Adolphus College and Minnesota State University, Mankato with a Bachelor’s degree in Psychology and a Master’s degree in Rehabilitation Counseling. He has been working with individuals with disabilities since 2005 in different capacities.

Laura Hokeness
Laura Hokeness graduated from UW Stout with a Bachelor of Science in Vocational Rehabilitation in 2004, a Master of Science in Vocational Counseling and Evaluation in 2006 and has been working in the rehabilitation industry since 2006. Laura has a Commission on Rehabilitation Counselor Certification (CRCC), Certified Vocational Evaluator (CVE) and Qualified Rehabilitation Consultation.

Michele Sward
Michele graduated with a bachelors from the University of Illinois in Applied Health Science with a concentration in Rehabilitation and Disability Studies and a Masters from University of Wisconsin- Madison in Rehabilitation Psychology. Michele has been a Qualified Rehabilitation Consultant since 2016 helping clients navigate the workers’ compensation system.

Britny Polasek-Hovde
Ms. Polasek-Hovde has been working as a QRC since 2013. She has a Masters from the University of Wisconsin-Madison (Go Bucky!) in Rehabilitation Psychology. She enjoys spending time with her family and raising her busy young daughter and German Shepard

Aerika Tori
Aerika Tori has BS in psychology and biology from the University of Minnesota. She participated in multiple research projects assessing orthopedic injury intervention outcomes and the relationship between the stress response and immune system. Aerika has been working in disability case management since 2018 and began as a Qualified Rehabilitation Consultant (QRC) in 2020. She is currently working toward a master's degree in rehabilitation counceling at St. Cloud State University. Aerika is detail oriented, timely, friendly and thorough which allows her to generate swift and positive outcomes in vocational rehabilitation.
Jaime Berry
Jaime is an administrative assistant at VCMN, balancing her busy life with
three children. Together, they enjoy traveling and outdoor activities. With
three dogs at home, Jaime also loves fostering dogs, sharing her passion for
animals with her family.
Bobbie Short
Bobbie Short enjoys working as an Administrative Support Specialist at VCMN. Bobbie retired from teaching and coaching to stay home and raise her four children. Bobbie enjoys spending time with her now grown children and herd of Grandkids. Bobbie is active with yoga, walking, playing golf and spending time at her cabin in Webb Lake, WI.

Catherine Burkhardt
Catherine Burkhardt graduated with her bachelor's degree in Social Work from
the University of Iowa with a concentration in Aging and Longevity Studies, and a minor in dance. She then graduated with her master's degree in social work from the University of Iowa just one year later. Catherine started as
our job placement specialist in August of 2023 and has enjoyed assisting clients find gainful employment. In her free time Catherine loves to teach dance and spend time with her family.
